Wellbeing and Social Prescribing

 

 

At Finney Lane Surgery we are keen to help you with anything that is impacting on your wellbeing and happiness.

In addition to medical support, we have a dedicated Wellbeing Team that supports patients with all non-medical issues and can put you in touch with useful local services and groups.

It is a non-urgent service which can help you with real health and wellbeing issues that can affect your life on a daily basis. It is not a medical or mental health service but can help you with underlying issues that affect your health.

A social prescriber is someone who:

  • you can talk to confidentially
  • gives you one to one support to help you manage your own health and wellbeing
  • can link you to local services and support

Issues the service can support you with:

  • long term conditions e.g. asthma, obesity, diabetes, anxiety, hypertension
  • mental wellbeing
  • housing problems
  • financial issues
  • social isolation
  • relationships
  • employment
  • lifestyle e.g. diet, physical activity, weight management

Alternatively, you call reception and ask for a referral to the Social Prescribing team.
