To register with the surgery:

We also may ask for ID or proof of address, but if you don’t have this you can still register. Your nationality or immigration status also do not matter; we will not require immigration documents to register patients.

If you require any support during the registration process or are having difficulty completing the form, please contact us after 10am Monday – Friday.

If you are a refugee or asylum seeker, please let us know upon registering so we can provide you with an initial health assessment questionnaire.

 

Catchment area

Before filling out a registration form, check you are in our catchment area.

 

If you are not within our catchment area, find out which GPs you can register with on the NHS website.

 

Named GP

All patients at Finney Lane Surgery have a named, accountable doctor who is responsible for coordinating their care.

Your named doctor will be allocated to you by the practice. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.

If you have a preference and would like to request a particular doctor at the practice to be your named GP please talk to one of our receptionists.
