The NHS does not pay for some of the services we provide. These include private sick notes, insurance forms, holiday cancellation forms, medical reports, fitness to travel certificates, private prescriptions, passport signing and some vaccination services. Our fees for these services are displayed in the surgery. The practice will aim to complete your requests within 28 working days following receipt of your payment, however there may be circumstances where we have to extend this timeframe, we will of course keep you updated on this.

Subject Access Requests
If you require access to a copy of your entire medical records, you will need to make a subject access request. If a legal representative requires this, in the majority of circumstances the records will provided directly to the patient, who can then provide this to the representative.
The time frame for completion of a subject access request is one calendar month. In exceptional circumstances, we may need to extend this to 3 months however if this is the case, we will inform you within one calendar month of the date of the request.
